Severe Spondylolisthesis(Grade 3 and 4) of the L5-S1, degenerative Spondylolisthesis with objective evidence of neurological impairment, fracture, dislocation, scoliosis, kyphosis, spinal tumor and failed previous fusion (pseudarthrosis).
